Was crafting Dragon Kanzir with the related materials through Mos, on channel 15 in Leafre. Upon clicking through to craft the item, my materials were taken, but I didn't receive a Dragon Kanzir. Attached screenshot for proof . I did and do have plenty of room in Equip and Etc tab to receive item prior to crafting. Also got stuck in CWKPQ Fire/Stirge Stage even though my party was able to move ahead, but I suppose that's a separate issue.
I don't know about the CWKPQ issue you're facing, but your issue with Mos is not a bug. If you craft using forging stimulators, Mos tells you there is a 10% chance you will not succeed. So what happened is intended, but unfortunate.
