I have been in three Zakum runs where once an Eye of Fire has been dropped, Zakum didnt spawn. After looking at the events priors to these runs, I am almost certain it has to do with a player or pet picking up the EOF once it has been initially dropped. I would highly recommend players to not drop the eye of fire to spawn zakum unless you are completely ready.
I have tested this out and can confirm that this is the case. Basically, don't drop the EOF before you're ready, but if you do get bugged this way, I've at least found a quicker and easier way for a GM to help you other than warping you all to another channel. (Will post this info up for the Staff to read.)
