I'd say probably the rarest (upgraded) item in the game would probably be a 47 att Christmas snow fur lump cape. 2 att from 30% scroll, then 9 perfect +5 chaos scrolls.

Rarest clean item is probably the perfect mark of naricain, like Kenny said.

 

i would think that cs +5 all stat for all 20 slots on a perf clean auf helm would be more rarer, u know aside from the once per year for lump vs many reloots u could hv in a year of auf helm

 

In case of cs potential, fur lump is rolling 1 dice 9 times, auf helm is rolling 4 dice 21 times,
they are not within the same order of magnitude.
